Social Worker Careers

Social workers specialize in providing social work case management, care coordination, transitional care interventions and community resource referral for typically for patients in NCQA recognized Medical Homes who are referred to the Community Health Team, primarily those with chronic conditions.  The CHT Social Worker may also provide outpatient mental health services; individual, group and family therapy. Levels of education and skills vary, with some positions requiring up to a Master’s Degree in Human Service Field such as Psychology, Mental Health Counseling, Social Work or Human Services.

Jobs for Pharmacists

As a Pharmacist at Rutland Regional you will assume a variety of clinical and distributive functions.  The pharmacist will be responsible as a proactive health care team member, for striving to identify, resolve and/or prevent potential and/or actual drug related problems.

We require our Pharmacists to be a graduate of an accredited school of pharmacy with a Bachelor of Science degree or PharmD. We prefer previous experience as a Registered Pharmacist in a hospital. We also require a Vermont pharmacy license or eligibility for licensure by examination or reciprocity.

Schedules vary covering day and evenings with rotating weekends and holidays.
